Established in 1994, The Polk Education Center offers specialized nutrition and lifestyle education on a variety of topics including diabetes, heart disease, stroke, weight loss, kidney disease, hypertension and more. The center provides education for both adults and children. As a certified diabetes education center, The Polk Center specializes in Diabetes Self Management Training. This type of training is the most successful program for maintaining blood sugar levels within the target zone which can reduce or eliminate the harmful effects of diabetes. The center is certified by the American Diabetes Association. Located on the first floor of CHI St. Luke's Health Memorial Lufkin, the center is staffed with trained professionals who develop individualized treatment plans designed specifically with you in mind.
