Diabetes Self-Management Training (individual)

Established in 1997, the Ascension Saint Thomas Health Diabetes Centers offer specialized nutrition and lifestyle education on a variety of topics including diabetes self-management education, pre-diabetes education, and medical nutrition therapy. These centers provide programs for adults at all stages of their diabetes journey. The training that we specialize in delivering is the most effective program for understanding how to control your diabetes, maintain blood sugar levels within your target range, and reducing or eliminating the long-term complications associated with diabetes. With a dual focus on individualized nutrition therapy and personal goal achievement, we will empower you to control your diabetes in a way that meets your lifestyle. Our centers are certified by the American Diabetes Association, and are located on the on the campuses of Ascension Saint Thomas Hospital Midtown in Nashville and Ascension Saint Thomas Rutherford Hospital in Murfreesboro. These centers are staffed with experienced Certified Diabetes Educators who will work with you to develop individualized treatment plans, self-management techniques, and the confidence needed to successfully overcome this challenge.
