- Facility feeoperating and recovery room services & supplies for outpatient stay (less than 24 hours) at Mesa View Regional Hospital
- Physician feetreatment and procedure-related postoperative follow-up visit(s) within 90 days of surgery. Does NOT include the initial evaluation to determine the need of the procedure.
- Anesthesiologist feeregional or general anesthesia for the entire duration of the procedure
- Pathology feespecimen (s) examination and tests for diagnosis. This test does not include the pathologic workup of malignancy. If a malignancy is discovered during your procedure, additional charges will be incurred.
- Tendo Marketplace feeCovers the cost of contracting upfront rates with all involved medical providers, processing payment, and distributing funds, minimizing administrative overhead and allowing for a lower total price

Dr. Ofori has spent a great deal of time talking to me about my condition. He treats me like a person while I am in his office which is an unusual thing in a doctor's office. He always remembers what is going on with me without doing five minutes of research in front of me. My overall health has improved since he became my OBGYN.
